E-Rate

E-rate funding involves several steps and the submission of specific forms to the Universal Service Administrative Company (USAC), the organization responsible for administering the E-rate program. The exact forms required may vary depending on your specific situation and the type of services you are requesting, but here are some common forms often used in the E-rate application process:

Form 470 - Description of Services Requested and Certification Form:

This form is used to request services, describe your needs, and invite service providers to bid on your project. It's an important starting point to indicate your intent to participate in the E-rate program.

Form 471 - Funding Request Form:

This form is used to request E-rate funding for the services you've selected. It includes detailed information about the services, service providers, and the costs involved. You need to file this form to be considered for funding.

Form 486 - Receipt of Service Confirmation and Children's Internet Protection Act (CIPA) Form:

After your funding request is approved, you need to submit Form 486 to confirm that services have begun and that you are in compliance with CIPA, which mandates internet safety measures for schools and libraries.

Form 499 - Universal Service Fund (USF) Information Sheet:

This form is unrelated to E-rate funding but is required to ensure you're registered for USF contributions, which help fund the E-rate program.

Form 472 - Billed Entity Applicant Reimbursement (BEAR) Form:

If you are paying the full cost of services upfront and seeking reimbursement, you can use the BEAR form to request reimbursement for eligible expenses.

Form 474 - Service Provider Invoice (SPI) Form:

If your service provider is receiving E-rate funding directly, they would submit this form to USAC to request payment for the discounted portion of your services.

Form 486 Notification Letter:

A service provider may need to file this letter to confirm that they have received a Form 486 from a client.

Form 500 - Amendments to FCC Form 471:

Use this form to make corrections or updates to your Form 471 after it has been submitted.
